On September 13, 2006, the FDA received reports of an unusual cluster of E. coli O157:H7 illnesses across multiple states. By September 14, the agency had issued a public warning: "preliminary epidemiological evidence suggests that bagged fresh spinach may be a possible cause of this outbreak." Within days, a critical link was confirmed: a bag of Dole baby spinach from the refrigerator of a sick New Mexico resident tested positive for the outbreak strain. The strain's genetic fingerprint — its PFGE pattern — matched the strain in sick patients across the country. Natural Selection Foods, the packing company, issued a voluntary recall. But by then, the spinach had already reached households, restaurants, and cafeterias in 26 states.
The FDA's final investigation, released March 2007, traced the contamination to a specific 50-acre farm in San Benito County, California — a spinach field leased by grower Mission Organics from Paicines Ranch, an Angus cattle operation. Among those samples: creek water, a wild boar, and four pieces of cow manure. The CIDRAP account of the environmental investigation notes that "the presence of wild pigs in and around spinach fields and the proximity of surface waterways to irrigation wells" were identified as the probable contamination pathway. Wild boar had broken through fences into and around the spinach field, leaving feces that carried E. coli O157:H7 into surface water that fed the irrigation wells supplying the spinach operation.
The spinach grew in contaminated soil and was irrigated with contaminated water. It was harvested, washed at the processing facility in San Juan Bautista, sealed in bags, and shipped. The washing — labeled "triple-washed" — did not eliminate the pathogen. E. coli O157:H7 can adhere to leaf surfaces and penetrate leaf tissue; washing with water reduces surface contamination but does not eliminate it. When the bags were sealed, the contamination was sealed inside with them — in product that looked, smelled, and tasted like safe salad. The first victim to die was Ruby Trautz, 81, a retired registered nurse from Bellevue, Nebraska, who died August 31, 2006. Three deaths total were confirmed. Thirty-one people developed HUS — hemolytic uremic syndrome, the kidney-destroying complication of E. coli O157:H7 that kills some of those who develop it.

Think of leafy greens as biological sponges exposed directly to their growing environment. When contaminated water touches spinach leaves — whether from irrigation, rain splash, or flooding — the bacteria don't just sit on the surface. E. coli O157:H7 can adhere tenaciously to leaf surfaces through a process called biofilm formation, physically bonding to the waxy leaf cuticle. More critically, it can enter leaf tissue through natural openings called stomata — tiny pores the plant uses for gas exchange. Once inside the leaf tissue, washing with water cannot reach it. The 2006 spinach outbreak demonstrated this directly: "triple-washed" product tested positive at the destination. The washing step that produces a "triple-washed" label is designed to remove physical soil and debris — it was not designed as a pathogen kill step, and it does not function as one for E. coli O157:H7 that has bonded to or entered the leaf. The only reliable kill step for E. coli O157:H7 in food is heat — cooking to at least 160°F internal temperature. Salad spinach is not cooked. There is no kill step between the field and your fork.

The Salinas Valley is simultaneously the source of a large fraction of the US's leafy greens and a region where cattle ranches and produce farms share a landscape, watersheds, and wildlife corridors. The specific contamination mechanism in 2006 — wild boar breaching fences and defecating in and around spinach fields — is a specific version of a broader wildlife and livestock proximity risk that persists wherever beef cattle and ready-to-eat produce operations share territory. It also accelerated the development of FDA's produce safety regulations under the Food Safety Modernization Act (FSMA), specifically the Standards for the Growing, Harvesting, Packing, and Holding of Produce for Human Consumption (the Produce Safety Rule). Before 2006, there were essentially no federal regulations governing how fresh produce was grown with respect to water quality, wildlife management, or field sanitation. The Salinas Valley had been producing leafy green-associated E. coli outbreaks for 13 years before the 2006 spinach event. The proximity of livestock operations to produce fields, the shared watersheds, the wildlife corridors — none of this was unknown. The specific risk factors were documented in prior outbreaks. The 2006 outbreak was the one that finally forced regulatory response, but it was not the first signal that the produce-livestock proximity in the Salinas Valley was a food safety hazard.
The "triple-washed" claim on bagged spinach labels describes a processing step: the greens are washed three times before packaging, typically with chlorinated water. This step removes physical debris, surface dirt, and reduces surface bacterial loads significantly. What it does not do, and was not designed to do: eliminate E. coli O157:H7 that has bonded to leaf surfaces or penetrated leaf tissue through stomata. The FDA's guidance since 2006 explicitly states that consumers should not re-wash pre-washed salad greens — not because the product is safe, but because re-washing a contaminated product in a home sink may spread the contamination to other produce and surfaces without reducing it on the leafy green itself. Who is most at risk
The 2006 spinach outbreak produced 31 cases of hemolytic uremic syndrome — a complication rate notably higher than typical E. coli outbreaks. HUS occurs when E. coli O157:H7's Shiga toxins enter the bloodstream, attack red blood cells (destroying them — hemolytic), and deposit debris in the tiny blood vessels of the kidneys, cutting off blood flow and causing acute kidney failure (uremic syndrome). In 5-10% of E. coli O157:H7 infections, HUS develops — most often in children under 5 and the elderly. HUS can cause permanent kidney damage requiring lifetime dialysis. The three deaths in the 2006 outbreak and most of the 31 HUS cases were adults — the usual E. coli O157:H7 HUS risk demographic applies here: not just children, but anyone with a less robust immune response to the Shiga toxins.

You cannot control what happens on the farm. You can make informed decisions about produce handling, risk awareness, and who in your household faces elevated risk from fresh produce.
FDA guidance since the 2006 outbreak is explicit: do not re-wash pre-washed, bagged leafy greens. Re-washing a contaminated product in your kitchen sink does not reduce the pathogen level on the greens, but may spread contamination to your sink, your hands, and other produce.
