Spring 1993. 403,000 people in Milwaukee drank treated, chlorinated city water and got sick — the largest waterborne disease outbreak in documented US history. At least 69 died, most of them people living with AIDS. The filtration plant was contaminated with Cryptosporidium parvum: a parasite that is resistant to chlorine. Standard water treatment doesn't kill everything. And what it doesn't kill is most dangerous for those who can least afford to be sick.

In early spring of 1993, hospitals and schools in Milwaukee began reporting unusual staffing shortages. Nurses and teachers were calling out sick. Two Milwaukee newspapers ran stories about a mysterious illness sweeping the city. The city's Department of Public Health began receiving reports that something was wrong. Within a two-week period, the explanation emerged: approximately 403,000 residents of the greater Milwaukee area were experiencing acute watery diarrhea — Cryptosporidiosis — caused by a microscopic parasite called Cryptosporidium parvum that had contaminated the city's southern water treatment plant.

The New England Journal of Medicine's study of the outbreak — which remains the definitive peer-reviewed account — documents the specific failure: "There were marked increases in the turbidity of treated water at the city's southern water-treatment plant from March 23 until April 9, when the plant was shut down." Turbidity measures the cloudiness of water caused by suspended particles. When turbidity rises above normal in treated water, it indicates that the filtration process is not removing particles adequately — including, in this case, Cryptosporidium oocysts. The plant used chlorination as its primary disinfection method. Chlorine is highly effective against bacterial pathogens like E. coli and Salmonella. It does not inactivate Cryptosporidium oocysts. The organism that contaminated Milwaukee's water in 1993 had been resistant to chlorine all along — but no one was monitoring specifically for it, because the standard treatment framework assumed chlorination was adequate for water safety.

The Healio account of the outbreak documents the death toll: at least 69 people died. The UWM Encyclopedia of Milwaukee entry is more specific about who died: "At least sixty-nine people — mostly people suffering from AIDS — died in this Cryptosporidium outbreak." For immunocompetent people, Cryptosporidiosis is a self-limiting illness — uncomfortable watery diarrhea that resolves within a week or two. For people with HIV/AIDS, cancer patients on chemotherapy, organ transplant recipients, and others with compromised immune systems, Cryptosporidiosis is uncontrollable and fatal: the diarrhea doesn't stop, nutrition cannot be absorbed, and the patient dies from dehydration and malnutrition. The Milwaukee outbreak's death toll was concentrated precisely in the population least able to survive waterborne illness — and that population's tap water was certified as safe.

A boil water advisory was issued on April 7, 1993 — after the outbreak had already peaked. The plant was shut down on April 9. The Milwaukee Slideshare analysis documents the public health response: "$96 million in healthcare costs and productivity losses." The city subsequently spent "upwards of $510 million in repairs, upgrades, and outreach to citizens" over the 25 years following the outbreak. The outbreak triggered a fundamental revision of US drinking water treatment standards: the EPA's Enhanced Surface Water Treatment Rule, the Long Term 1 and 2 Enhanced Surface Water Treatment Rules, and the requirement for Cryptosporidium-specific monitoring in large water systems all trace directly to the Milwaukee 1993 outbreak. The standard treatment framework that had been in place was exposed as insufficient. It was redesigned. But the redesign came after 403,000 people got sick and at least 69 died.

What makes Cryptosporidium uniquely dangerous in water systems

Think of water treatment as a two-step process: filtration removes particles (including pathogens), and disinfection (chlorination) kills remaining organisms. Cryptosporidium exploits a gap between these two steps. Its oocysts — the dormant, environmentally resistant stage of the parasite's life cycle — are small enough to pass through a poorly functioning filtration system. And once they're in the water, chlorine at standard treatment doses does not inactivate them. The Waslix / Water Quality and Health Council analysis states it directly: "In 1993 the chlorine-resistant parasite Cryptosporidium parvum outbreak affected over 400,000 people." The Milwaukee plant's filtration was failing during the outbreak period — turbidity levels were rising — but even if filtration had been perfect, the chlorine backup would not have provided protection against this specific organism.

Why the immunocompromised face a categorically different risk from waterborne pathogens

The NEJM analysis of the Milwaukee outbreak documents the clinical reality of Cryptosporidium's differential effects: "In immunocompetent people, cryptosporidiosis is a self-limited illness, but in those who are immunocompromised, infection can be unrelenting and fatal." For a healthy person, Cryptosporidium causes watery diarrhea for 1–2 weeks and then the immune system clears it. For a person with HIV/AIDS (particularly those with low CD4 counts), the immune system cannot clear the infection. The diarrhea becomes chronic — losing liters of fluid per day. Malabsorption prevents nutrition. The patient progressively deteriorates. This is not a matter of degree — it is a categorical difference in how the same organism affects different people. The 1993 Milwaukee deaths were concentrated in the HIV/AIDS population precisely because that community could not tolerate an infection that the general population experienced as a temporary nuisance.

What the Milwaukee outbreak changed in US water safety regulation

The Milwaukee outbreak is the regulatory benchmark for Cryptosporidium in US drinking water. Before 1993, there were no specific monitoring requirements for Cryptosporidium in treated drinking water, and the standard treatment framework assumed that chlorination was adequate disinfection. After Milwaukee, the EPA developed a series of targeted rules: the Enhanced Surface Water Treatment Rule (1998) required improved filtration to reduce Cryptosporidium; the Long Term 1 and 2 Enhanced Surface Water Treatment Rules (2002, 2006) specifically required large water systems to monitor for Cryptosporidium and, if concentrations exceeded thresholds, to implement additional treatment (UV disinfection or ozone, which does inactivate Cryptosporidium). The Filtration Failure

March 23 – April 9, 1993: Turbidity in treated water at the Howard Avenue (southern) Water Purification Plant rises above normal. Early April: Hospitals and schools report unusual numbers of people calling out sick. Two Milwaukee newspapers cover "a mystery illness." City Department of Public Health becomes concerned. NHL hockey teams and college hockey players in town for NCAA tournament leave Milwaukee carrying the parasite without knowing.

April 7: Boil water advisory officially issued to the public. April 9: Howard Avenue plant shut down. By this point: approximately 403,000 people infected over approximately 2 weeks. 4,400 hospitalized. At least 69 dead — primarily people with AIDS, elderly, immunocompromised. The regulatory gap

No Cryptosporidium monitoring before the outbreak

In 1993, there were no federal requirements for monitoring Cryptosporidium in treated drinking water. The regulatory framework assumed that properly operated, chlorinated water was safe from protozoan pathogens. This assumption was wrong for Cryptosporidium specifically — but no one was required to check for it. The rising turbidity was visible in plant monitoring data, and turbidity is an indirect indicator of filtration failure. But the specific pathogen that was passing through the failing filter was not being monitored. The gap between "the filter is failing" and "here is what is passing through the failing filter" cost 403,000 people their health and at least 69 their lives.

The 17-day window when the plant should have been shut down

The turbidity in treated water at the Howard Avenue plant rose above normal beginning March 23. The plant wasn't shut down until April 9 — 17 days later. The boil water advisory wasn't issued until April 7 — 15 days into the documented filtration failure. During those 17 days, approximately 403,000 people continued drinking contaminated water. The failure to act immediately on elevated turbidity readings is one of the specific operational lessons that the post-Milwaukee regulatory changes addressed: turbidity limits became enforceable treatment technique requirements, not just monitoring values.

When a boil water advisory is issued, follow it fully: boiling, cooking, ice, teeth brushing, and produce

A boil water advisory means all tap water should be boiled before: drinking, making ice cubes, washing raw produce, brushing teeth, and cooking. Many people understand "don't drink the water" but continue to make ice from the tap, brush their teeth with unboiled water, or wash fruit. These are all exposure pathways. Understand that not all water filters remove Cryptosporidium — choose NSF-certified products specifically rated for cysts

Standard water filtration pitchers (Brita-style, activated carbon) do not reliably remove Cryptosporidium oocysts.
