The Mayo Clinic's guidance on anaphylaxis is direct: untreated, it can cause death within thirty minutes, and clinical research on emergency department cases finds that the first hour after exposure accounts for roughly half of all anaphylaxis fatalities. A written allergy list does not prevent the reaction. It prevents the version of the emergency where a provider who has never met you accidentally causes one.
The scenario the list exists for is specific: a person arrives confused, unconscious, or unable to speak, and the people treating them have no medical history to work from. Latex gloves, a contrast dye, a common antibiotic, or a medication given in good faith can trigger the exact reaction the rest of the care team is trying to prevent. A written list, kept somewhere a first responder will actually look, closes that gap before it opens.
- List the trigger and the reaction together, not just the trigger. "Penicillin" tells a provider less than "penicillin, causes throat swelling within minutes." The severity changes how cautiously they proceed with related drug classes.
- Cover all four major categories: medications, foods, insect stings, and contact materials like latex. A reaction history in any of these categories can change emergency treatment decisions, not just routine ones.

02 — The medical condition summary card: one document that answers the first six questions a provider asks
When a person cannot speak for themselves, the first minutes of care are spent gathering information that a card could have supplied instantly: who are you, what conditions do you manage, what do you currently take, and who should we call. A medical condition summary card answers all of it on one page.
The card serves two different audiences in two different moments. In the field, a paramedic uses it to avoid a dangerous drug interaction or recognize a known condition fast. In the emergency department, the same card speeds up triage by replacing a verbal history that the patient cannot give.
- Identity basics: full name, date of birth, and blood type, since blood type matters in any transfusion scenario and is rarely something a bystander can supply from memory.
- Active conditions and current medications with exact doses. "Takes blood pressure medication" is far less useful than the specific drug and dose, since dosage affects how a provider calculates safe additional treatment.
- Allergies, cross-referenced with the list from the previous section, so the two documents agree rather than contradict each other if someone finds both.
- Contacts that matter in the moment: an emergency contact, the primary doctor's name and number, and the insurance carrier, all of which a hospital intake process needs almost immediately.

03 — Immunization records: the specific reason a tetanus date matters more than whether the shot ever happened
CDC guidance on tetanus prevention sets a precise threshold that most people don't know: a booster is needed if more than five years have passed since the last dose and the wound is dirty or contaminated, compared with the standard ten-year interval for routine boosters. A debris injury during disaster cleanup is exactly the kind of contaminated wound this five-year rule is written for, which means the date of the last shot, not just whether it was ever given, determines what a provider does next.
Tetanus enters through broken skin, and the kind of injuries common after a storm, downed branches, splintered wood, twisted metal, debris with embedded dirt, are precisely the contaminated-wound category the shorter interval exists for. Without a documented date, a provider has to default to giving the booster regardless, which is a reasonable but avoidable precaution if the actual date were on hand.
- Record the date of the last tetanus-containing dose, not just "yes, vaccinated." Td and Tdap both count; what matters for the five-year rule is the date of the most recent one.
- Include children's full immunization history, since school enrollment, camp, and travel can all require records on short notice, and the original paper card is easy to lose during a household disruption.
- A documented record avoids both over- and under-treatment. Without it, a provider may give a redundant dose out of caution, or a patient may decline a needed one because they assume they're current when they aren't.
04 — Medical device documentation: the model number, not just the device name, is what makes backup power possible
An oxygen concentrator and a CPAP machine are both medical devices, but one draws 300 to 600 watts running continuously and the other draws 30 to 90 watts running several hours a night. Sizing a backup power source, a generator, a battery bank, or a portable power station, depends entirely on knowing which device and which model is actually in use, not the general category.
The HHS emPOWER Program exists because this problem is large enough to be a federal public health concern: more than three million Medicare beneficiaries rely on electricity-dependent durable medical equipment, and a prolonged outage is genuinely life-threatening for that population. emPOWER itself works at the community level, helping emergency managers and utilities plan for at-risk populations in aggregate. The household-level equivalent is the same idea applied to one home: documentation specific enough that backup power can actually be sized correctly before an outage, not figured out during one.
- Record device name, manufacturer, and model number. The model number is what a supplier or battery manufacturer needs to confirm compatibility, not the generic device category.
- Record power draw in watts, not just "it runs on electricity." The number is usually printed on the device label or the power adapter, calculated as volts multiplied by amps. If it isn't listed, the user manual or the manufacturer's support line will have it.
- Note that draw can vary with settings. An oxygen concentrator's flow rate and a CPAP's humidifier setting both change power consumption, so size backup power to the highest expected draw, not the average.
- Include the supplier's contact information, since durable medical equipment suppliers can often provide backup battery guidance, loaner units, or portable oxygen cylinders specific to the model in use.
Devices that cannot tolerate interruption
Ventilators and some infusion pumps cannot safely lose power even briefly. For any device in this category, documentation should include not just specifications but an explicit relocation trigger, the point at which remaining battery runtime means moving to a hospital or shelter rather than continuing to wait out the outage at home.
05 — Pet care instructions: the same continuity problem, with a different subject
If a household member is evacuated, hospitalized, or otherwise unavailable, a pet still needs feeding, medication, and basic handling on schedule. The ASPCA's planning guidance calls the resulting document a pet portfolio, and its core insight is simple: whoever steps in, a neighbor, a relative, a boarding facility, needs the same kind of structured information a human caregiver would need, just for an animal who cannot describe its own needs.
This is the same documentation problem as the rest of this page, applied to a dependent who cannot speak for itself. A feeding schedule, a medication list, and a vet's contact information turn an unfamiliar animal into a manageable one for whoever takes over, even on short notice.
