From Three Mile Island to Chernobyl, the pattern repeats. What protects people is the same each time: get inside, stay inside, and listen to official guidance. Here is what you need to know before the alert arrives.
The Scenario
On March 28, 1979, a pressure relief valve at the Three Mile Island nuclear plant in Dauphin County, Pennsylvania, stuck open for more than two hours. Cooling water drained from the reactor core. Operators, reading instruments that showed the valve as closed, did not understand what was happening. By the time they identified the problem, the reactor's upper core had partially melted. The accident had been unfolding for hours before it was understood at all.
No one died directly from Three Mile Island. An estimated 140,000 people — primarily pregnant women and children under five, on the recommendation of the Nuclear Regulatory Commission — voluntarily evacuated a five-mile radius. The surrounding population stayed in place. A small amount of radioactive gas was released. Fear, confusion, and a near-total failure of official communication caused as much harm as the radiation itself.
That was 1979. The world has 440 operating nuclear reactors today. Forty-six operate in the United States. Beyond reactors, radioactive materials are used in hospitals, industrial equipment, and research facilities in every major metropolitan area. A nuclear or radiological emergency is not a Cold War abstraction. It is a scenario that local emergency managers in 35 states currently plan for actively.
This guide does not cover nuclear war. It covers the realistic spectrum: plant accidents, industrial source incidents, and the dispersal device (dirty bomb) scenario. The protective actions for all three overlap significantly — and they are not complicated.
How It Escalates
Unlike a tornado or earthquake, most nuclear incidents have a warning window. That window is where household preparation matters most.
An alert goes out. WEA (Wireless Emergency Alert) and EAS (Emergency Alert System) are the primary channels. The message will specify shelter-in-place, potassium iodide distribution, or evacuation depending on location and event type. Most households within the 10-mile Emergency Planning Zone (EPZ) will be told to shelter first. Grocery stores near the plant sell out of water within an hour. Traffic on evacuation routes begins building within two hours.
Sheltering reduces exposure dramatically but is not permanent protection. Households sheltering in place will run through what they have on hand: food, water, medications, battery power for devices. Official information is often delayed, contradictory, or technically framed in ways that confuse rather than clarify. Three Mile Island's lesson: the information environment is usually the hardest part. Having a confirmed official source before the event matters enormously.
By the end of the first week, the NRC and state public health agencies will have ground-level radiation readings across affected zones. Evacuation orders will either be lifted, extended, or expanded. The 50-mile ingestion pathway zone becomes relevant — this is where food and water restrictions apply (dairy, produce, surface water). People living within 50 miles of a plant but outside the 10-mile EPZ will face guidance about tap water, local produce, and well water safety.
Chernobyl resulted in a permanent exclusion zone. Three Mile Island did not require any permanent relocation. The outcome depends on what was released and how much. Most U.S. reactor accident scenarios — based on NRC modeling — do not produce a Chernobyl-level release. The lingering challenge is psychological: uncertainty about long-term health risk is a documented source of sustained stress. The communities near Three Mile Island showed elevated anxiety and stress-related health outcomes for years after the accident.
Vulnerability
Biology, location, and access to information determine who faces the highest risk.
Developing cells are more sensitive to radiation than adult cells. The thyroid gland in children is especially vulnerable to radioactive iodine, which is why the NRC's guidance prioritizes potassium iodide distribution for those under 18 first. This is not a precaution, it is a radiation biology fact established by decades of post-Chernobyl research.
The NRC's Emergency Planning Zone is not arbitrary. Residents within this radius face the highest probability of needing protective action. They are the target of the most specific emergency plans, the fastest alert systems, and the first potassium iodide distribution. If you live within 10 miles of a nuclear facility, your county's emergency management office has a specific plan for your address.
For households within 50 miles of a plant, an ingestion pathway event restricts local produce, dairy, and surface water. People who depend on well water, backyard gardens, or locally produced dairy face disruption to primary food sources. This applies to a much larger population than most assume — the 50-mile ingestion zone from a single large plant can encompass millions of households.
Know which of your immediate neighbors have children under five or are pregnant. In an event where KI is being distributed, they need to get to a distribution point first. Elderly neighbors who may not receive or process wireless alerts also need someone checking on them. The protective action is the same for everyone, but the speed and completeness of executing it varies by circumstance.

The cascade in a nuclear event is different from most disasters. The physical plant itself may be contained early. What fails is everything built around public confidence in information.
Three Mile Island's most studied failure was communication. The NRC, the utility (Metropolitan Edison), Pennsylvania Governor Richard Thornburgh, and President Carter all issued different guidance within 48 hours. Residents had no way to know which source to follow. Modern WEA and EAS systems have improved alerting, but the fundamental challenge of translating radiation risk into plain-language protective action has not been fully solved. The single most protective thing a household can do is decide in advance which official source they will follow: their state's emergency management agency website and official app.
Every NRC-required emergency plan includes evacuation route maps for the 10-mile EPZ. These routes are publicly available through your county emergency management office. Spontaneous evacuation — people leaving before or without an official order — is the primary cause of route congestion. At Three Mile Island, approximately 144,000 people left without being asked to. Knowing your official evacuation route in advance means you are not improvising in a traffic-jammed, cell-service-degraded environment.
A reactor release that produces radioactive iodine and cesium can contaminate surface water, forage, and produce within the 50-mile ingestion pathway zone. Municipal water treatment is not designed to remove radioactive contamination — stored water from before the event is safe; water sourced during the event may not be. This is the failure point for households without any stored water supply.
What surprised people at Three Mile Island
The utility's public statements minimized the event while regulators were privately describing it as a serious incident. Residents who monitored multiple official channels simultaneously received contradictory information for 72+ hours. The stress of uncertainty drove voluntary evacuation more than the actual radiation levels did. Studies conducted in the years after found no statistically significant increase in cancer rates near the plant — but did find lasting increases in psychological distress and cardiovascular symptoms consistent with chronic stress.

Dauphin County, PA · 1979
Partial meltdown, small radiation release, no direct deaths. The event demonstrated that communication failure can produce worse outcomes than the hazard itself. The NRC was restructured, emergency planning requirements were overhauled, and the 10-mile and 50-mile EPZ system was formalized directly in response.
What worked: immediate NRC on-site response within hours. What failed: coordinated public messaging for 72+ hours.
Chernobyl, Ukraine · 1986
The world's worst commercial nuclear accident: full reactor meltdown, graphite fire, massive radiation release over 10 days. Thirty-one workers and firefighters died in the immediate response. An estimated 350,000 people were eventually evacuated. The Chernobyl Exclusion Zone remains uninhabited today. No U.S. reactor design or operating procedure permits the conditions that caused Chernobyl; it requires a different reactor type (RBMK) that is not used in the United States.
What failed: suppression of information for 36 hours while the fire burned. Swedish radiation detectors discovered the event before the Soviet government acknowledged it.
Goiânia, Brazil · 1987
A disused medical radiotherapy unit was scavenged from an abandoned hospital. Workers opened the source capsule, found a glowing blue cesium-137 powder, and shared it with family and neighbors who found it beautiful. Four people died. 249 were contaminated. The incident established international protocols for radioactive source tracking and is the model case for the "orphan source" scenario — radioactive materials outside regulatory control. It is not a hypothetical in the United States: the NRC tracks over 1,000 lost, stolen, or abandoned radioactive sources per year.
What worked: eventual decontamination of an entire urban neighborhood. What failed: no mechanism existed to track a source that had been abandoned for years.
What You Can Do Now
Three of these take less than twenty minutes and cost nothing. Two require a small amount of stored material. All five are endorsed by the NRC, FEMA, and the CDC.
The NRC's website lists every U.S. commercial reactor and its location. Your county emergency management office has a specific emergency plan for residents within the 10-mile EPZ. Request the evacuation route map for your address. This takes ten minutes, costs nothing, and is the single most useful action for anyone near a plant.
Nuclear risk in your areaEffective sheltering means reducing air exchange with the outside. Close and lock all windows and doors. Turn off HVAC, exhaust fans, and fireplace dampers. Move to an interior room on a middle floor — exterior walls and the roof are the least effective barriers. A brick or concrete building reduces outdoor radiation exposure by a factor of 10 or more. An interior room in a wood-frame house reduces it by a factor of two. Both are better than outside.
First 72 hours emergency guideStored water from before the event is uncontaminated. Surface water, tap water, and well water sourced during or after a radiological event may be restricted. At Three Mile Island, restrictions on well water in the surrounding county lasted weeks. Storing a minimum of one gallon per person per day, sealed in food-grade containers, removes the most common post-event water access problem entirely.
Water storage guidePotassium iodide (KI) floods the thyroid with stable iodine, blocking uptake of radioactive iodine-131 — one specific isotope released in some reactor accidents. It does not protect against any other form of radiation. It is not a general radiation antidote. If you live within 10 miles of a nuclear plant, your county may distribute KI tablets for free. Taking KI without direction from public health authorities is not recommended — and is actively harmful for people with thyroid conditions.
