Generator Safety for Medical Users
When someone in the household depends on a powered medical device, the pressure to bring the generator closer is enormous. That pressure kills people. Here is how to power your equipment safely.

Why this page matters more than most
Carbon monoxide from portable generators is one of the leading causes of poisoning death in the United States.
The CPSC estimates that about 85 people die in the United States each year from carbon monoxide poisoning caused by portable generators.1 Most of those deaths happen in homes during power outages. Between 2011 and 2021, at least 770 people died this way.2 A single portable generator can produce carbon monoxide concentrations comparable to hundreds of cars.1
For households that depend on a generator to power medical equipment, the risk is heightened by the very thing that makes the generator necessary: the urgency. When an oxygen concentrator needs electricity and the power is out, the instinct is to bring the generator as close to the house as possible, to run it in the garage with the door open, to get power flowing fast. That instinct is the exact pattern that kills people.
This guide covers the non-negotiable safety rules, how to set up a safe configuration in advance, and how to protect your household with carbon monoxide alarms. The non-negotiable rules
Three rules that do not bend for medical need
Carbon monoxide is colorless, odorless, and tasteless. You cannot see it, smell it, or taste it. People who are sleeping or who have been weakened by illness can die from CO poisoning before they experience any symptoms.3 The following rules come directly from the CPSC and the CDC. No medical circumstance overrides them.
Never run a generator indoors
Never use a portable generator inside a home, garage, basement, crawlspace, shed, or any other enclosed or partially enclosed space. Opening doors or windows does not provide enough ventilation to prevent lethal CO buildup.1,4
This is the rule that kills people when broken. The CPSC reports that 81 percent of generator-related CO deaths occur in residential locations.1 A garage with the door open is one of the most common fatal configurations. The convenience of a short extension cord is not worth a life.
Keep it at least 20 feet from the house
Operate generators outdoors only, at least 20 feet from homes and buildings, with the exhaust directed away from windows, doors, and vents.1,4,5 This is the minimum distance recommended by both the CPSC and the CDC.
Twenty feet is about six adult paces. Measure it once, mark the spot, and run a heavy-duty outdoor-rated extension cord from there to the house. Set this up before storm season, not during the emergency. A properly rated cord loses very little power over 20 feet.
Install and maintain CO alarms
Install battery-operated CO alarms or CO alarms with battery backup on every level of the home and outside each sleeping area.1,3,6 The CPSC and the EPA both make this recommendation. The alarm must work during a power outage, which is the exact time you are most likely to be running a generator.
Test CO alarms before each storm season. Replace them according to the manufacturer's schedule, typically every five to seven years. Never ignore a sounding CO alarm. If the alarm goes off, everyone leaves the building immediately and calls 911 from outside.3
Before storm season
Set up the safe configuration now, not during the emergency
The time to figure out where the generator goes, how the cord runs, and which outlets it feeds is a calm afternoon in September, not midnight during a hurricane. The medical-user version of this setup is the same as the general version with one additional layer: the cord must reach the room where the medical device operates, and the device must be tested on generator power before the outage.
Choose and mark the generator location
Walk 20 feet (about six paces) from every window, door, and vent on the house. Find a flat, dry spot at or beyond that distance where the exhaust will point away from the building. Mark it. A painted line, a paving stone, a post: anything that says "generator goes here" when it is dark and raining and you are stressed.
Avoid locations near dryer vents, HVAC intake vents, crawlspace openings, or attic soffit vents. CO can enter the house through any of these paths even from 20 feet away if the exhaust is pointed toward them.
Run the cord
Buy a heavy-duty outdoor-rated extension cord long enough to reach from the generator location to the room where the medical device runs. The cord should be rated for the wattage you need. For medical devices, a 10-gauge or 12-gauge cord rated for 15 to 20 amps handles most situations. Check the label.
Run the cord through a window or door that can be mostly closed around it. Close every other window and door near the generator to minimize CO infiltration. Do not run the cord through a space that blocks your exit path.
Test with your medical device
On a calm day, run the full setup: generator at the marked location, cord run to the device room, medical device plugged into the cord and operating. Run it for long enough to confirm that the device works normally on generator power, that the cord does not overheat, and that the generator's output is clean enough for your equipment (pure sine wave output is required for most medical devices). This test run is the same practice recommended in the backup battery guide. The system you have never tested is the system you do not know works.
Consider a generator with CO safety features
The CPSC recommends asking retailers for portable generators with built-in CO detection and automatic shutoff.7 Some newer models have engines that produce substantially less carbon monoxide than older designs. These features add a layer of protection but are not a substitute for proper outdoor placement. A CO shutoff is a backup safety net, not permission to move the generator closer to the house.
Recognizing the danger
Know the symptoms of carbon monoxide poisoning and what to do
Carbon monoxide poisoning is dangerous precisely because its symptoms mimic common illness. During a power outage, when people are already stressed, tired, and possibly unwell, CO symptoms can be mistaken for the flu or exhaustion. Every member of the household who is old enough to understand should know these symptoms and the response.
Symptoms of CO poisoning (CDC)3
Headache, dizziness, weakness, upset stomach, vomiting, chest pain, and confusion. These symptoms are often described as "flu-like." CO poisoning can progress quickly to disorientation, loss of consciousness, and death.
People who are sleeping, under the influence of alcohol, or weakened by illness can die from CO poisoning before they experience any symptoms. This is why a CO alarm is not optional: it is the only warning system that works when people cannot sense the danger themselves.
If you suspect CO poisoning
Leave the building immediately. Do not stop to turn off the generator. Do not open windows first. Get everyone, including pets, outside into fresh air.
Call 911 from outside. Tell the dispatcher that you suspect carbon monoxide poisoning and that a generator was running.
Do not re-enter the building until emergency services have cleared it. If someone has collapsed and you can safely remove them, do so without entering deeply into the structure. Carbon monoxide accumulates at floor level and in enclosed spaces, and a rescuer who enters an affected building can also be overcome.
